I have seen a lot of people who have had surgery where there has been a mistaken attempt to refine the tip of the nose by removing too much cartilage from the tip or in order to thin the nose, have had too much cartilage removed from the side wall of the nose. So in this situation, it can cause a pinched tip look. By removing the structural support of the sidewall, the rims of the nose can retract upward, the tip can retract upward, breathing problems can occur, and so these are the things that are addressed in revision surgery. It is possible to take someone who has had a really horrible result and revise the nose by restructuring both the functional and cosmetic parts so that the nose has a more natural appearance. Sometimes it is not possible to say lower the tip of the nose if it has been over-rotated, but it is possible to give the illusion that the tip has been lowered by doing other things such as raising the columella or mid portion of the nose. Likewise, it is possible to lower the rims of the nose by sewing cartilage grafts into this area. Although we cannot always undo all the changes that have been made that are not to the patient’s benefit, it is possible to make other changes to compensate for these things.
