It is a fallacy, however, to believe that by continually tightening and over-tightening the face, a person can maintain or regain a youthful appearance. Much of the appearance of aging has to do with a loss of fullness in the face. It is this fullness that gives the face a youthful appearance. In women, he places these behind the tragus (the small flap of cartilage that sticks out directly in front of the ear canal). In men, he places them just in front of the tragus. Dr. Schwartz has often seen cases of poor incision planning in men, in which the hair-bearing skin has been brought up onto the tragus. As a result, this area requires shaving following facelift surgery, an extremely inconvenient if not impossible task.

Dr. Schwartz is careful to not elevate the earlobe during facelift surgery and to replace the skin without tension in order to achieve the least noticeable scar possible. By keeping the earlobe in its normal anatomic position, and not over-resecting the skin, the earlobe will not take on a tethered appearance, one of the telltale signs of a poorly performed facelift. He is also extremely careful in his incision planning and closure so as to not distort the hairline behind the ear and to avoid scarring in this area as much as possible.
